When first adding a new symptom from your Care Plan, look for the text "Link to existing health conditions" in the "Add Symptoms" screen or the "Edit Symptoms" screen. You may select your condition from the drop down list to associate a symptom to a condition. If you need to add a new condition, tap "More" from the bottom navigation bar, tap My Conditions, and add a new condition.
